The club was founded in 1949 and spent most of its years in the lower leagues. In 1969, the team qualified for League A, the second league at the time, and played six consecutive years in the minor league. The team's record achievement was fourth place, where it finished in the 1971/1972 and 1972/1973 seasons.

 

At the end of the 1975/1976 season, Hapoel Tirat Carmel was ranked ninth. In that season, the first qualification in each district for the National League (which was the top league), and the teams that finished in 2-5th place in each district advanced to the national league that was established as the new minor league. Hapoel Tirat Carmel remained in League A, which became the Third League at the end of that season, but one season later qualified for the National League and survived there for three seasons, until it was relegated back to League A at the end of 1979/1980 season.

 

In 1989, Hapoel Tirat Carmel returned to the national league after an absence of nine seasons, under the guidance of coach Eli Gutman. The team survived in the National League for only two seasons, until it was relegated back to League A at the end of the 1990/1991 season.

 

In 1995, Hapoel Tirat Carmel was relegated to League B, and after another season, it dropped to League C. In its first season in League C, the team tried to qualify back to League B, but finished in second place, two points less than Po'al Dalit Al-Karmel, and following the failure, the team was disbanded.

 

In 2013, the team was re-established by Shimon Peretz and Idan Elias, and registered in the third league. At the end of the 2015/2016 season, the team qualified for the second league.
